Medicaid Buy-In by state

Working-disabled Medicaid pathways can allow a disabled person to earn above ordinary Medicaid limits and keep coverage, sometimes with a premium. States set their own financial and operational rules.

2026 snapshot, not an eligibility decision. The figures below use KFF's April 2026 state survey. CMS describes Buy-In as optional state eligibility groups and advises workers to contact their state Medicaid agency. States can change limits, disregards, premiums and program rules.
